2022-2023 School Year 

Returning families should fill out the Returning Students Application.

• Returning Students Application (Due March 18th, 2022)
• Monarch Montessori 2022-23 Contract Due March 25th, 2022(This is provided after conferences)

The following forms must be completed and returned to the office before your child’s first day of school.

• Medication Authorization Form
• Health Inventory
• Immunization Certificate
• Emergency Form

2022-2023 School Year and beyond

Our programs have limited enrollment. There is a waiting list of interested families. We encourage you to contact us as early as possible. Usually the December-February prior to a September School start.
